POUCZENIE / INSTRUCTION / INSTRUCTION


1) Należy wypełnić wszystkie wymagane rubryki. W przypadku braku niezbędnych danych
wniosek może być pozostawiony bez rozpatrzenia.
All required fields should be completed. In case of lack of necessary data the application might not be
reviewed.
Il faut remplir toutes les cases demandées. En cas d’absence des données nécessaires, la demande pourrait
ne pas être instruite.

2) Kwestionariusz należy wypełnić czytelnie, drukowanymi literami wpisywanymi w odpowiednich
kratkach.

The application should be completed legibly, in printed letters entered into the appropriate boxes.

Le formulaire doit être rempli lisiblement, en majuscules inscrites dans les cases correspondantes.

3) Części A, B, C, D i F wypełnia wnioskodawca.

Parts A, B, C, D and F should be completed by the applicant.

Les parties A, B, C, D et F sont à remplir par le demandeur.

4) Część E wypełnia organ przyjmujący wniosek.

Part E shall be completed by the authority accepting the application.

La partie E est à remplir par l’organe qui reçoit la demande.


5) W części A, w rubryce „Płeć” wpisać M – dla mężczyzny, K – dla kobiety; w rubryce: „Stan
cywilny” należy użyć sformułowań: panna, kawaler, mężatka, żonaty, rozwiedziona,
rozwiedziony, wdowa, wdowiec, wolna, wolny.
In Part A in the „Sex” field „M” should be entered for a male and „K” for a female; in „Marital status”
fields the following terms should be used: maiden, bachelor, married, divorced, widow, widower, single.
Dans la partie A, rubrique „Sexe” – inscrire: „M” pour un homme, „K” pour une femme; rubrique „état
civil”, utilisez une des formulations suivantes : célibataire, marié, mariée, divorcée, divorcé, veuve, veuf,
libre.

6) W części F podpis nie może wychodzić poza ramki.

In Part F the signature should be within the box.

Dans la partie F, la signature ne doit pas dépasser l’encadrement.

7) W przypadku gdy wniosek ma obejmować dzieci i inne osoby podlegające wpisowi do
dokumentu, należy dodatkowo wypełnić części A i C wniosku dla każdej z tych osób, podając
stopień pokrewieństwa.
In case of the application is supposed to include children and other persons subjected to enter them into the
document, in addition parts A and C of the application should be completed for each of these persons, with
the degree of relationship.
Si l’invitation doit englober les enfants et autres personnes à inscrire au document, les parties A et C
doivent être remplies séparément pour chacune de ces personnes, avec l’indication du degré de parenté.
